We are seeking an experienced Nurse Practitioner to provide evidence-based primary and family care in a collaborative clinical environment. The provider will be responsible for patient assessment, diagnosis, treatment, chronic disease management, health counseling, and care planning while working closely with a multidisciplinary healthcare team.
Location: Norwich, Connecticut, USA
Employment Type: Full-Time
Schedule: Minimum 2–3 days per week, 8:30 AM – 7:30 PM. Additional hours may be available.
Key Responsibilities
- Provide comprehensive primary and family care, including assessment, diagnosis, treatment, and management of acute and chronic health conditions.
- Deliver preventive care services, health screenings, wellness counseling, and patient education.
- Develop and coordinate patient care plans with healthcare professionals to improve patient outcomes.
- Conduct routine examinations and diagnostic evaluations.
- Encourage patients to actively participate in their healthcare and wellness goals.
- Perform administrative duties, including clinic opening/closing procedures, inventory management, patient follow-up, insurance verification, payment collection, and maintaining a clean clinical environment.
- Manage patient appointments and documentation accurately using Electronic Medical Record (EMR/EHR) systems.
Required Qualifications
- Master's Degree from an accredited Family Nurse Practitioner program.
- Current National Board Certification.
- Active and unrestricted Nurse Practitioner license in the state of practice.
- Current BLS certification through the American Heart Association (AHA) or American Red Cross (ARC).
- Minimum 1 year of recent experience as a Nurse Practitioner in:
- Primary Care
- Family Medicine
- Urgent Care
